This position is responsible for providing a variety of health information management/medical records functions. 

 

The work involves responsibility for data entry into the electronic health record including clinical diagnoses and tools and scanning documents. Maintains confidentiality of protected health information according to HIPAA, CFR 42, and state mental health laws. Processes correspondence and release of information requests. Monitors documentation for completeness and performs quality assurance activities. The position requires a “customer friendly” service orientation in a behavioral health setting utilizing the Robert Young Center’s Trauma Informed Care Guiding Principles

Responsibilities

Data Entry 

  • Preps and scans paper documents into the electronic health record 
  • Enters notes on appropriate documents and forms 
  • Enters clinical data from the record into fields in the electronic health record, including diagnosis, Ohio scales, allergies, and substance history 
  • Processes Psychiatric evaluations and assigns appropriate ICD-10 code to record 
  • Enters data into state required databases including Ohio Scales 
  • Analyzes and processes transfer and closing documents and update electronic health record accordingly 
  • Processes daily admissions and discharges, opening or closing the case to Psychiatric Inpatient or Riverside programs in the electronic health record

Correspondence 

  • Processes PCP (primary care physician) referral and school forms 
  • Receive and process requests for information according to state and federal regulations 
  • Receive and process court order and subpoena requests according to regulations 
  • Obtains records from other agencies per request of therapist or physician 
  • Monitors outstanding request and on hold lists 
  • Assists walk-in or phone call client requests in a timely, courteous and professional manner 
  • Properly log incoming and outgoing requests for information in the electronic health record for accounting of disclosures 
  • Retrieves archived files for requests of information on microfilm 

Deficiency Analysis 

  • Analyzes medical records for completeness of documentation, signatures and dates 
  • Verifies and updates completion of physician electronic signatures and dictation by monitoring dictation reports and the “No Matches” in the electronic health record 
  • Assists the therapists and physicians with the chart completion process 
  • Monitors and records data for QA activities 

 

Qualifications

Education: High School graduation (or GED)
